A Service Coordinator provides case management to each adult and child enrolled in a program that serves persons with intellectual/developmental disabilities or autism. The Service Coordinator assists with enrollment and ongoing services planning and monitoring to persons receiving community resources and Medicaid waiver programs. Service Coordinators also help people when they are in crisis.
